Position Summary
The Coordinator, Access & Flow in partnership with the Access and Flow Manager, is responsible for monitoring and providing direction to bed allocation team and supporting the day to day flow of patients across the organization, including facilitating timely transfers between units. This role liaises with the interdisciplinary team daily including nursing, social work, physicians, allied health, and Charge Nurses to facilitate timely discharges. Responsible for communication and liaising with physician partners, patient care units and patient service partners, internal and external. Receives and coordinates Repatriations and Inter-Facility transfer requests daily, which is a primary responsibility of this role. Responsible for leading bed meetings twice daily, as well as providing the key stakeholders with an end of day report. This role is instrumental in inputting ALC data into ALCIS. In collaboration with ED CN and access and flow manager identifies Bed Alerts as required. Identifies pre grid lock status while utilizing Bed Management Policy and Discharge Planning policy. Conducts regular rounds on units to maintain visibility.
Mandatory Qualifications
Regulated health care professional (Nursing, Physiotherapy, Occupational therapy or social work) with 3-5 years of clinical experience in an access and flow environment
